Sports begin in the end of August. The final deadline to submit an annual sport physical to the nurse is August 6, 2010. The physical must be recorded on the Garden City Medical Evaluation form, with the student’s health history completed and signed by a parent. This form can be picked up in the Main Office or downloaded from the Garden City High School website- Nurse’s Corner. No one will be available after August 6th to review physicals for completeness. DO NOT send green Athletic Health History forms to the school!  Your child must bring the green form with them on the 1^st day of tryouts.
